If you would like to drive for your classroom field trip, the attached form needs to be completed and submitted to the main office, along with a copy of your driver's license, vehicle registration, and vehicle insurance.

If your child needs medication (Tylenol, Ibuprofen, Albuterol, Benadryl, EpiPen, cough drops, etc.) at school, please complete and sign the attached form and submit it, along with your child's medication, to the main office.  Please note that each medication needs its own form.
